The Change of Broker form and the License Status Update form have been combined to create the Change of Broker/License Status Update form.
Using this new form, you can:
Inactivate your license, change your Broker, reactivate your license with your previous Broker, or reactivate your license with a different Broker.
All instructions, fees, and how to submit the form (and any other required documentation) are all located on the form near the action you are wishing to complete.
